Access to Daycare

St Luke’s Home Day Care Centre is funded by the HSE – South.

Access to our Day Care Centre is through the Public Health Nurse (PHN). Your local Health Center or General Practitioner will be able to inform you who your PHN is and how to make contact with them. The PHN will visit you in your home to assess your needs and fill in their part of the Day Care application – the remaining section needs to be completed by your GP. When the application is received your name will go on a waiting-list for Day Care and a place will be allocated as soon as possible.

Once a place has been allocated for St Luke’s, our Day Care Nurse Manager will contact you to arrange a suitable day and advise on available transport. A small daily charge, set by the HSE, is collected from all Day Care clients each time they visit.

St. Luke’s Day Care Centre serves Sector 6 of the South Lee area of Cork (this includes Mahon, Blackrock, Ballintemple, Douglas Road, Ballinlough and into the City as far as Abbey Street). Clients with dementia may apply if living anywhere in Cork city or suburbs although priority is given to those living locally.

Further information can be obtained from the Day Care Nurse Manager in the Home on 021-4359444.
